Our Mission
The mission of Path To Wellness is to improve the health and well being of members living in marginalized communities through access to healthcare, mental health and substance use counseling, education, support and advocacy.
Our Vision
At Path To Wellness, we understand that caring for the whole person is complex and individualized. We believe that addressing a broad range of human needs is the best way to improve a person’s health.
All aspects of the health care education program we provide are non-dependent on insurance. It is our goal to provide preventative health care.
We are proud to partner with other community organizations that align with our mission of caring for the underprivileged communities in Cook County.
