Health Education Sessions
We offer health education sessions provided by registered nurses, advanced practice nurses, and physicians. This also includes providing access to dietitians who teach healthy cooking classes and reading food labels.
Health Care Screenings
We offer health care screenings to assist residents in identifying issues such as hypertension, diabetes, cardiac issues, and depression, and provide access appropriate care.
Social Workers
We work with a network of licensed clinical social workers who can assist residents to identify appropriate resources with warm hands-off to medical professionals as required or requested. This will also include assisting with transportation needs to screening exams and doctor’s appointments.
Mobile Crisis Teams
Mobile crisis teams comprised of a variety of health care professionals (M.D.s, APRNs, CADCs, Neuropsychologists) provide assessments for physical, social, emotional and substance use problems. The crisis team assessment is more intensive than a basic health care screening and the results will be shared with the individual’s medical support team if one exists and contingent upon approval from the resident. We also provide 24/7 access for our crisis hotline and to our crisis management team.
